Please complete this form to give formal notice that you wish to end your membership.

Submitting this form does not immediately end your membership or cancel any payments. We will review your membership agreement and contact you by email to confirm:

The date your cancellation notice was received
Your confirmed membership end date
Any remaining payments due under your current membership commitment
When your membership benefits and access will end

There is a one month notice period to terminate membership.

Cancellation requests take effect from the date they are received and cannot be backdated.
